Purpose:
To provide residential medical care designed to manage
patient’s care at home and to decrease hospitalization
and avoid patient placement outside of the home.

Skilled nursing to teach patient’s family how to
manage medical conditions while offering ongoing
assessment of patient’s condition.

Call” nursing 24 hours a day

Physical Therapy to maintain/improve patient’s
mobility

Occupational Therapy

Speech
Therapy

Home
Health Aide to assist with personal hygiene care and
activities of Daily Living

sources: Medicare, Medicaid, Private Insurance,
Self-Pay
Qualifier:
A skilled service must be ordered by the patient’s
Physician. Patient must be homebound.
Referrals:
A referral can be received by anyone. The patient’s
Physician must sign the plan of care, certifying that
the patient is homebound. If you are
leaving the hospital, ask your discharge planner or
nurse about what Home Health Services are available.
